Director of Social Services - Falmouth

Sedgewood Commons located in Falmouth, is a Specialized Memory Support Community offers individualized care for those living with Alzheimer's Disease or other forms of dementia. Family involvement is very important to us. Each person's family is an integral part of the care team. By providing education and keeping family members informed, our goal is to enhance interactions and support these meaningful connections to promote an optimal quality of life. In January 2017, Sedgewood Commons opened a new 26-Bed Rehabilitation Unit for those living with Alzheimer's Disease or other forms of dementia. The responsibilities of the Director of Social Services includes a key role within our interdisciplinary team, case management, discharge planning, advocacy, providing support to residents and family members, providing information and conducting tours for people considering placement, and regulatory compliance. Licensed Social Worker - Full-time
