Program Description: Creating a defensible medical record that communicates the necessary details of resident care requires training and understanding of the challenges and pitfalls of documentation. With critical thinking skills as the foundation, this presentation addresses documentation guidelines, the difference between subjective and objective documentation, risky words/phrases and alternatives, out-of-the-ordinary entries, and legal terms including spoliation and contributory negligence. One of the most difficult areas to document is when a resident or family has a complaint; this presentation will offer best practices in this particular area. We’ll discuss the drawbacks of forms and electronic medical records and how to incorporate critical thinking skills to mitigate some of those areas of risk.
