Care Plan Development & Interdisciplinary Team Meetings
CMS requires skilled nursing facilities to accurately assess each resident and develop a comprehensive care plan with instructions for providing effective, person-centered care that meets professional standards of quality care. This session will review regulatory requirements related to care planning. We will discuss practical approaches leaders can implement immediately to improve resident/family satisfaction, enhance quality of care, and ensure survey compliance.
Learning Objectives:
- Define basic principles of care planning and person-centered care planning.
- Recognize regulatory requirements and MDS 3.0 guidelines related to care planning.
- Identify key resources for developing a person-centered care plan.
- Describe an effective Interdisciplinary Team Meeting process to support accurate person-centered care planning.
