MDS Medicare Case Coordinator

Position Summary

The MDS Coordinator is responsible for clinical oversight and coordination of person-centered quality care at Cove’s Edge in collaboration with the Director of Nursing.  This includes clinical operations, as well as fiscal and regulatory requirements.  Responsible for coordination, completion and submission of MDS and review of CMS reports of the MDS forms, Care Area Assessments and other related assessments.  Coordinates IDT meeting schedule and responsible for care planning.  Key player in continuous quality improvement program and efforts.  Communicates the importance of Cove’s Edge philosophy, as well as LincolnHealth Mission and Vision.  Emphasize the importance of the employee/customer relationship to realize positive outcomes.  Acts as liaison with other departments to ensure care is delivered collaboratively using an interdisciplinary team approach.  Member of the Cove’s Edge Clinical Operations team.

Requirements

Required Minimum Knowledge, Skills, and Abilities (KSAs)

    1. Education: Minimum associate's degree or equivalent from two-year accredited School of Nursing. 
    2. License/Certifications: Current Maine State Registered Professional Nurse license required. Current BLS certification required. Certificate in Gerontology and Nursing Management preferred. AANAC MDS Certification preferred.
    3. Experience: Minimum six months to one year of related experience and/or training (including geriatric nursing and nursing management). Experience long term care nursing strongly preferred. Experience with RAI process, MDS, Medicare/Medicaid reimbursement coordination strongly preferred. Experience in dementia care strongly preferred.
    4. Familiarity with and support of the Culture Change movement strongly preferred.
    5. Ability to read, analyze, and interpret the most complex documents.  Ability to respond effectively to the most sensitive inquiries or complaints from customers, regulatory agencies, or members of the business community.  Ability to effectively present information to management, public groups, employees, customers and/or boards of directors.
    6. Ability to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions, and decimals.  Ability to compute rate, ratio and percent.  Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations.
    7. Ability to define problems, collect data, establish facts, and draw valid conclusions.
