MDS/Care Plan Nurse Manager
Borderview Rehab & Living Center, healthcare facility located in Van Buren,
has an immediate opening for MDS/Care Plan Nurse Manager.
 
Duties will include managing the entire MDS process, completing assessments
and developing care plans.


 The ideal candidate will have:
· A friendly, team-oriented disposition
· Flexibility, and be detailed oriented
· Understanding of PDPM
· Experience in a long term care or rehabilitation setting
· RN required
· Experience with the MDS is preferred but not required
· State of Maine License
