Redefining Rehab in Senior Care: Innovative Solutions for Value-Based Care
This webinar examines how rehabilitation can move beyond traditional service delivery to become a strategic contributor to value-based performance in skilled nursing and senior living communities. The presentation explores emerging hybrid care models that combine in-person therapy with telehealth, remote therapeutic monitoring, and AI-assisted technologies to extend reach, improve clinical insight, and optimize resource utilization. It also highlights innovative therapy programs tailored to senior care environments that support value-based care initiatives. Attendees will leave with a clearer understanding of how modern rehab approaches can drive measurable outcomes while supporting operational efficiency.
 
By the end of this presentation, participants will be able to:

  • Explain the role of rehabilitation in advancing value-based care within skilled nursing and senior living settings
  • Identify the benefits of hybrid rehabilitation care models that utilize telehealth, remote therapeutic monitoring and AI assisted technologies

Presenter
Rachael Percoco, MSPT, DPT, has extensive experience and a deep passion for healthcare, which has established her as a prominent figure in healthcare conferences. She has contributed to the development of curriculum models at Northeastern University and the American Physical Therapy Association. Her expertise in addressing complex topics has garnered a strong following among peers throughout the Northeast, including various state associations, physician groups, medical teams, and community forums. Since joining Preferred Therapy Solutions in 2015, Rachael has served as the Director of Outpatient Services. She is a Physical Therapist with a Master of Physical Therapy from Northeastern University and a Doctorate in Physical Therapy from Simmons College. Rachael is also among a select group of physical therapists clinically certified in pelvic floor therapy. In addition to her clinical work, she owns a holistic physical therapy practice and holds certifications in yoga and Tai Chi. Rachael is dedicated to providing everyone with the educational resources and tools they need to achieve their optimal health goals.
