Updated SNF QRP Quality Measure Calculations and Reporting User’s Manual V6.0 and Associated Resources

The Skilled Nursing Facility (SNF) Quality Reporting Program (QRP) Quality Measure Calculations and Reporting User's Manual Version 6.0, along with related documents, is now available. These resources provide updated specifications for calculating quality measures included in the SNF QRP, effective as of October 1, 2024.
 
The SNF QRP Measure Calculations and Reporting User's Manual Version 6.0 includes revisions to the specifications for quality measure calculations. Additionally, the accompanying Change Table Version 6.0 outlines the key updates.
 
CMS has also released the following updated resources for SNF QRP measures:
Risk Adjustment Appendix File: Includes risk-adjustment values for calculating risk-adjusted quality measures.
Crosswalks of Hierarchical Condition Categories (HCC) to ICD-10 Codes: Used in the risk-adjustment models for Discharge Self-Care, Discharge Mobility, and Discharge Function Score measures.
Imputation Appendix File: Provides imputed coefficients for calculating the risk-adjusted Discharge Function Score measure.
