CMS to Reformat Nursing Home Statements of Deficiency

The Centers for Medicare & Medicaid Services (CMS) will soon reformat nursing home Statements of Deficiency (CMS Form-2567) to list the most severe survey findings first, rather than ordering them alphabetically or by F-tag. Announced by David Wright, Director of the CMS Quality, Safety & Oversight Group at the 2025 Quality Conference, the change aims to make critical issues more visible and improve clarity in lengthy reports.

Initially focused on nursing homes, the new format will prioritize deficiencies by scope and severity. This update supports CMS's broader push for transparency, including a recent policy requiring deficiencies to be posted as soon as providers receive them, without waiting for correction plans or appeals.

CMS is also exploring the use of artificial intelligence to extract actionable insights from deficiency data and reviewing public input to identify outdated or burdensome participation requirements, with the goal of increasing flexibility and innovation across the healthcare system.
