DHHS Vaccine Clinic Request Form and NEW Vaccination Rates by Zip Code

Long term care facilities are strongly encouraged to secure a relationship with local long term care/independent pharmacies to facilitate COVID-19 vaccination of new residents and staff. MHCA has reported extensively on these Maine CDC recommendations to providers. In addition, we are pleased to report that the Department is now taking requests for vaccine clinics. If your organization is interested in hosting a clinic for 10 or more people, please submit the form:
