Updated Application & Deadline for DHHS Relief Grants

Last week, DHHS updated the Hospital and LTC application for CRF relief grants to include a second reporting period (1/1/21 to 3/31/21). The form now includes a column for revenues, expenses, and occupancy applicable to 1/1/21 to 3/31/21.  The Department recognizes that this information for the 1st quarter of 2021 may capture the absence of direct federal funding, increased operational expenses, and/or a decrease in occupancy for providers. The reporting period (either 4/1/20-3/31/21 or 1/1/21-3/31/21) with the largest loss will be used in the allocation of the CRF financial relief grant. 

Additionally, DHHS is extending the due date: please submit the application by 5:00 pm on Friday July 9, 2021.  If you have already submitted your application, you are welcome to resubmit your application on the updated form.  DHHS encourages all hospitals, nursing facilities, residential care facilities-appendix Cs, and adult family care homes to submit an application.
