CDC Releases 2026–2027 COVID-19 Vaccine Clinical Considerations

The CDC has posted interim clinical considerations for the 2026–2027 COVID-19 vaccines, and Medical Directors, Infection Preventionists, and leadership teams should review them as they plan fall vaccination programs. Key resources include the overview of COVID-19 vaccines and vaccination, separate guidance for people who are not immunocompromised and those who are immunocompromised, implementation guidance, and contraindications and precautions. Because the right schedule depends on age, immune status, prior vaccine history, product, and whether a use is off-label, centers should not rely on a single simplified dosing schedule. Work with your medical director and providers to determine the appropriate schedule for each resident; staff should consult their own healthcare provider. The CDC guidance also identifies when a use is off-label per the FDA. AHCA has confirmed with CMS that off-label use for residents will continue to be covered by Medicare and is working with CMS to make sure the MACs and other contractors follow that policy.
As a reminder, F-887 requires nursing homes to document that residents (or their representatives) and staff were educated on the benefits and potential side effects of COVID-19 vaccination and were offered the vaccine or information on how to get it. Records should show each person's vaccination status and, if not vaccinated, the reason, such as refusal, prior vaccination, or medical contraindication. Keep the dates and materials used for education, maintain resident and staff records, and track coverage rates to guide ongoing outreach. Maine's standing order for COVID-19 vaccine administration is available from Maine CDC, and AHCA/NCAL's #GetVaccinated campaign offers tools to support your respiratory season efforts.
