New Process for Requesting PPE and Testing Supplies in Maine

Below are details for you as providers to request supplies moving forward:
If your organization would like to request a shipment of Personal Protective Equipment (PPE), Nasopharyngeal Swabs (NP), or Viral Transport Medium (VTM) after July 26th, please use the following link to download the request form: PPE Request Form.
Once completed, the form should be emailed to the following address: PPEorC19TestingSupplies.MECDC@maine.gov
Please note that, after July 26, 2021, County EMA offices will no longer accept resource requests for PPE, NP, or VTM from organizations for submission to the Maine CDC warehouse fulfillment team. Instead, the resource request form must be emailed to the dedicated Maine CDC email inbox listed above for fulfillment.
The Maine CDC warehouse fulfillment team will process, pick, and ship your order using United Parcel Service (UPS), Department of Transportation delivery vehicle, or another government vehicle driven by a Maine CDC warehouse team member.
Testing supply requests will be filled on an as-submitted basis. PPE requests will be filled on a two-week cycle beginning July 26th. Please follow the directions contained in the form; incomplete forms cannot be processed. We expect that the turn-around and shipping time will be similar to the previous request system. Please do not wait until you are critically low on supplies before making a resource request. You should be prepared to answer a few questions about your request, should the order fulfillment team have any questions or concerns. Maine CDC will be unable to fill requests made outside of this process.
