SNF VBP Quarterly Preview Reports Now Available

CMS has released the June 2026 Quarterly Confidential Feedback Reports for the FY 2027 Skilled Nursing Facility Value-Based Purchasing (SNF VBP) Program. These reports are now available for download through iQIES.

What's in the Report?
Your facility-level results are included for eight quality measures covering your FY 2027 performance period:
  • SNF 30-Day All-Cause Readmission (SNFRM)
  • Healthcare-Associated Infections Requiring Hospitalization (SNF HAI)
  • Discharge to Community—Post-Acute Care (DTC PAC SNF)
  • Hospitalizations per 1,000 Long Stay Resident Days
  • Total Nursing Staff Turnover
  • Total Nurse Staffing Hours per Resident Day
  • Discharge Function Score
  • Falls with Major Injury (Long-Stay)
These results will be used to calculate your facility's performance scores and incentive payment multipliers, which take effect October 1, 2026. Final Performance Score Reports will be released in August 2026.

Why This Report Matters
This is the first FY 2027 SNF VBP Program year to assess performance on eight measures — double the previous four. Reviewing your results now gives your team time to identify areas for improvement before scores are finalized.

Correction Request Deadline: July 1, 2026

If you believe there is an error in your measure results due to a CMS calculation mistake, you have until July 1, 2026 to submit a correction request. Send your request to SNFVBPquestions@cms.hhs.gov with the subject line "SNF VBP Review and Correction Inquiry" and include your CCN, facility name, the specific correction requested, and your rationale.
