AHRQ Nursing Home Survey on Patient Safety Culture: Voluntary Data Submission

The Agency for Healthcare Research and Quality (AHRQ) will open its data submission window for the Surveys on Patient Safety Culture® (SOPS®) Nursing Home Survey from September 1-18. AHRQ, an agency within HHS, focuses on health services research aimed at making care safer and higher quality — and the SOPS Nursing Home Survey is one of the tools it offers facilities to help assess staff perceptions of patient safety culture, spot strengths, and identify opportunities for improvement.
It's important to note that this submission is entirely voluntary — not a CMS or state survey requirement. Nursing homes that have administered the SOPS Nursing Home Survey (Version 2.0 or 1.0), including the Workplace Safety Supplemental Items, between November 1, 2024, and September 18, 2026, may choose to submit their results through the SOPS Nursing Home Survey Database. All submitted data remain confidential, and facilities that participate receive a custom feedback report they can use for internal quality improvement work.
Facilities interested in participating should visit the submission website ahead of time to review guidance and tools. Questions can be directed to DatabasesOnSafetyCulture@westat.com or 1-888-324-9790.
