Excellence in LTC Nomination Form

Across Maine, dedicated healthcare workers provide vital care, supportive services, comfort and peace of mind to some of our most vulnerable citizens. Every year, in partnership with the Maine Long Term Care Ombudsman Program, MHCA looks forward to celebrating the best and brightest of these individuals at our Excellence in Long Term Care Awards.

From home care to adult day services programs to assisted living to nursing facility care, we know that caregivers are the backbone and lifeline for Maine’s older and disabled citizens. It is our pleasure to honor their dedication, performance and all the exemplary things caregivers do to provide high quality care, reassure loved ones, and truly make a difference in the lives of their residents/consumers.

Because many team members contribute to the quality of care and life, we present caregiving awards across all disciplines as follows:

1) Nursing/direct caregiver, such as a Nurse, CNA, PSS, CRMA, HHA, DSP, or other direct caregiver, AND
2) All other disciplines, like social work, dietary, activities, etc., from any other department

Facilities and organizations may submit one nomination in each category for a total of two nominations. All nominees must be employed by their organization/facility for at least one year. An independent panel of judges will select the winners, who will be recognized during a special live event on special live event on November 13 at Thomas College in Waterville, Maine.
