Submission Deadline for 2025 Excellence in Long Term Care Awards Extended to August 1!

Below you will find an overview of the questions on the online nomination form:
Section 1: Nominator's Contact Information / Relationship with Nominee
Section 2: Nominee's Name, Current Job Title, Past Job Titles (if any), Facility/Organization Name, Estimated Date of Hire, Nominee's Email address
Section 3: Description of why your nominee deserves this award (500 word limit). You may wish to include a quote from a resident, consumer, or family member that reflects the nominee's impact; specific examples of how the nominee has gone above and beyond during the past year, especially in the face of workforce challenges; ways the nominee demonstrates professionalism and serves as a role model to peers and supervisors; actions that promote resident or consumer quality of life and rights; and heartfelt or memorable moment that captures the nominee's commitment to caregiving.

MHCA and the Maine Long Term Care Ombudsman Program have partnered to present the 2025 Excellence in Long Term Care Awards to recognize long term care employees across the continuum, including those working in home health care, adult day care, assisted living, and nursing facilities. We will honor our national AHCA Quality Award recipients at the MHCA Fall Conference.
The Excellence in Long Term Care Awards recognizes employees statewide who consistently exemplify a commendable level of caregiving, dedication, and professionalism in caring for their residents and consumers. Because many team members contribute to the quality of care and life, we present caregiving awards across all disciplines as follows:
- Nursing/direct caregiver, such as a Nurse, CNA, PSS, CRMA, HHA, DSP, or other direct caregiver, AND
- All other disciplines, like social work, dietary, activities, etc., from any other department
Facilities and organizations may submit one nomination in each category for a total of two nominations. All nominees must be employed by their organization/facility for at least one year. An independent panel of judges will select the winners, who will be recognized during a special live event on November 13.
There's never been a better time to honor your caregivers. Nominating your staff requires a small investment of time and energy but can mean much to the recipient.
